Junior League Work Order Form

Provide a brief description of the project. If this is for an event, please include the event name and all relevant info we need. If this is a photoshoot or video, please give us as many details as possible...Number of people, how long if video, etc...
PLEASE NOTE - Photoshoots must be booked at least 30 days in advance, and video projects must be scheduled 60 days out to include filming, post production, and back and forth edits.
